Once completed this form will create a tracking ticket within our system, which you can use for further correspondence.
Authorization forms are required:
- For ECE faculty, provide a speedchart/worktag.
- For Researchers, send the Authorization Form filled by your supervisor to ECE Finance to start with the ECE administrative process.
Typically, the parts you requested should be ready for pick up within five business days. Once ready, you will be notified via email.
Note: If you do not receive an email within 72 hours, please reply to the confirmation email for an update.
